Are opioid prescriptions less common for patients who identify as Black, Hispanic or Latino, Asian, or Pacific Islander (PI) than non-Hispanic White patients following orthopaedic procedures in academic United States health systems? Of the patients receiving a postoperative opioid prescription, does analgesic dose differ between non-Hispanic White patients and Black, Hispanic or Latino, or Asian or PI patients, when stratified by surgical procedure type?
During the period spanning January 2017 and March 2021, 60,782 patients underwent an orthopedic surgical procedure at facilities within the Penn Medicine healthcare system, comprising six hospitals. Of the total patient population, 61% (36,854) were eligible for inclusion in the study, defined as those who had not been prescribed an opioid within the past twelve months. Excluding 40% (24,106) of the patients, this selection was based on their failure to undergo one of the eight most frequent orthopaedic procedures studied, or if the procedure was not conducted by a Penn Medicine faculty member. Records for 382 patients lacked race or ethnicity information, either due to omission or refusal, and were subsequently excluded from the analysis. Subsequent analysis utilized a cohort of 12366 patients. In the surveyed patient group, 65% (8076) of individuals identified as non-Hispanic White, 27% (3289) as Black, 3% (372) as Hispanic or Latino, 3% (318) as Asian or Pacific Islander, and 3% (311) as belonging to another racial group. For analytical purposes, prescription dosages were transformed into total morphine milligram equivalents. Multivariate logistic regression modeling, accounting for age, sex, and insurance type, was used to evaluate variations in postoperative opioid prescription patterns within procedure categories. The Kruskal-Wallis test was applied to examine the effect of procedures on the total morphine milligram equivalent dosage administered in the prescriptions.
Of the 12,366 patients, 11,770 (95%) received a prescription for an opioid medication. Following risk adjustment, no disparity was observed in the odds of Black patients receiving a postoperative opioid prescription, compared to non-Hispanic White patients (odds ratio 0.94, 95% confidence interval 0.78 to 1.15; p = 0.68). Similar results were found for Hispanic or Latino, Asian or Pacific Islander, and other racial groups. Postoperative opioid analgesic prescriptions, measured in median morphine milligram equivalents, did not vary by race or ethnicity, regardless of the eight procedures performed (p > 0.01 for each).
Our analysis of opioid prescribing practices in this academic health system following common orthopedic procedures revealed no variations based on patient race or ethnicity. An alternative explanation might be the application of surgical pathways in our orthopedic department. A reduction in variability of opioid prescriptions is a potential outcome of adopting formally standardized opioid prescribing guidelines.
